Registration Procedure (Registration)

To register for the conference and pay the registration fees, please complete the Registration Form and return it to the INFORMS SSC Secretariat Office by fax (+886-2-2730-3297),or email (hime@mail.ntust.edu.tw).

1. If you register through returning the Registration Form, we will process the payment with the credit card information you provide.
2. To whom pay by remittance:
Please wire the fees to the following account, and return the remittance receipt to us via fax (+886-2-2730-3297) or email (scan the receipt and send the file to hime@mail.ntust.edu.tw).

Guidelines for participants from the Mainland China area: (Visa Information)

1.      Please filled out "ApplicationForm99informs.doc" in Chinese and send it back to Hime <hime@mail.ntust.edu.tw>.  (You could either send the form back as a ".doc" file or as a PDF file.)

2.      Along with the application form, please also send us your

- handwriting signature
- passport size photo
- valid qualifications*
- copy of ID card issued by the PRC government
- copy of student or teacher ID (if applicable)

Please note that all files should be saved as either JPG or PDF file.

*Valid qualifications of your professional expertise or current job position (such as diploma, employment certificate, or certificate of enrollment)

3. "INFOMRSpackage.rar" consists of some documents you may need when you apply for the entrance permit to Taiwan.  To download the application form for entrance permit and the INFORMS package, please go to:
